Once upon a time, there was a king. The king liked one of his followers very much because he was very wise and always gave very useful advice. Therefore the king took him along wherever he went.
One day, the king was bitten by a dog, the finger was injured and the wound was getting worse. He asked the follower if that was a bad sign. The follower said, Good or bad, hard to say'. In the end, the finger of the king was too bad that had to be cut. The king asked the follower again if that was a bad sign. Again, the follower gave the same answer, 'Good or bad, hard to say'. The king became very angry and sent the follower to prison.
One day, the king went hunting in the jungle.. He got excited when he was on the chase of a deer. Deeper and deeper he went inside the jungle. In the end he found himself lost in the jungle. To make thing worse, he got captured by the native people lived inside the jungle.
They wanted to sacrifice him to their god. But when they noticed that the king had one finger short, they released him immediately as he was not a perfect man anymore and not suitable for sacrifice. The king managed to get back to his palace after all. And he finally understood the follower's wise quote, 'Good or bad, hard to say'. If he hadn't lost one finger,he could have been killed by the
native people.
He ordered to release the follower, and apologized to him.. But to the king amaze, the follower was not mad at him at all. Instead, the follower said, 'It wasn't a bad thing that you locked me up.' Why? Because if the king hadn't locked the follower up, he would have brought the follower along to the jungle. If the native found that the king was not suitable, they would have used the follower. Again, the quote 'Good or bad, hard to say' stands.
The moral of the story:
Everything that happens in this world, there is no absolute good or bad. Sometimes good things turned out to be bad things eventually, while bad things become a gain.
Whatever good things that happen to you, enjoy it, but don't have to hold too tight to it, treat it as a surprise in your life.Whatever bad things that happen to you, don't have to feel too sad or despair, in the end, it might not be a total bad thing after all.
If one can understand this, he or she will find life much easier.

Swine Influenza A (H1N1)
What is swine influenza?
It is a respiratory disease of pigs caused by type A strains of the influenza virus. It regularly causes high flu outbreaks in pigs but with low death rates. There are four main sub-types of the virus, but the most recent isolated influenza viruses from pigs have been H1N1 viruses.
How does it spread?
Swine flu viruses do not typically infect humans though they do occur through close proximity or contact with infected pigs or contaminated areas. Cases of human-to-human spread have been documented.
What are the symptoms?
The symptoms are similar to those of regular flu:
- Fever
- Lethargy
- Runny nose
- Cough
- Sore throat
- Lack of appetite
- Vomiting and diarrhoea in some cases.
How common is swine flu infection in humans?
In the past reports of about one human swine flu virus infection had been received every one to two years in the United States. From December 2005 till February 2009, 12 cases have been reported.

People with swine influenza virus infection should be considered potentially contagious as long as they are symptomatic; possibly for up to seven days following the onset of the illness. Children, especially younger children, might potentially be contagious for longer periods.
What can I do to protect myself from the swine flu?
There is no vaccine available right now to protect against the swine flu.
However, you can help prevent the spread of germs that cause respiratory illnesses like influenza by:
- Covering your nose and mouth with a disposable tissue or handkerchief when you cough or sneeze. Throw the tissue in the waste basket after you use it.
- Wash your hands often with soap and water, especially after you cough or sneeze. Alcohol-based hand cleaners are also helpful
- Try to avoid close contact with sick people. - If you get sick with influenza, stay at home and limit contact with others to keep from infecting them.
- Avoid touching your eyes, nose or mouth.
- Consult your nearest healthcare facility if you think you have any of the symptoms.

The World Health Organisation (WHO) has confirmed human cases of swine flu in Mexico, the United States, Canada and Spain. Only Mexico has reported deaths from the new strain.

The Health Ministry on April 27 advised Malaysians against travelling to certain places in the United States, Mexico and Canada which have been affected by the swine flu.
What precautions are in place in Malaysia?
- The Health Ministry’s operations room in Putrajaya has started a 24-hour monitoring of the situation. The public can call 03-8881 0200/300 for enquiries.
- Those returning from Latin American countries and found to have flu-like symptoms will be quarantined.
- Health Ministry officials are conducting health screenings on passengers arriving from the United States.
- Thermal scanners will be placed at international airports to speed up the screening process for swine flu.
- Public and private medical practitioners have been instructed to report to the district health office any patient with influenza-like illnesses or severe pneumonia symptoms and who had travelled to the affected countries after April 17.
- Owners of the 797 pig farms nationwide have been ordered to immediately contact the nearest Veterinary Services Department office or the Animal Disease Control Centre if their workers or animals show symptoms linked to the swine flu.

Today is 3rd day of d new year 2009
its been 3 days d in tis wonderful new year 2009... hahaha... d new year started 2 days ago n today is d 3rd day... still dunno wat to do n still enjoyin my holiday without doin anything interesting... sometimes its quit boring.. bt i kinda like it... at least no need to touch any books for tis 1 whole month... hahaha.. banzai...
bt d bad thing is if i din touch any books i may 4gt a lot of stuff... n i call myself a med student... haha.. med student r usually lz thou... nt talkin bout d others la of cuz.. i m talkin bout myself... tats y i m at least helpin out my bro in his clinic few days la so called in a week... at least thr is some refreshment of wat i hv been studyin all d while in 3 years...
tis year gonna be my brand new year in pursuing my ambition... tats to become a doc... i heard a lot anyhow... bout d tension in doin med course.. bt i still wanna be 1... haha... no matter wat trust me, its kinda challenging n interesting... if thr is no exam it would b wonderful la. bt how could it b possible... hahaha... so d exam tension will alwiz be thr.. hahaha... kinda stressful u noe if u dun study well... d feeling of getting a pass n d feelin tat u noe u may fail d paper...

The Korotkoff sounds n ways to measure BP
Korotkoff sounds - There are five sounds which are heard as the blood pressure in the sphygmomanometer cuff is released during the measurement of arterial pressure; these are described as:
• Korotkoff I is a sharp thud
• Korotkoff II is a loud blowing sound
• Korotkoff III is a soft thud
• Korotkoff IV is a soft blowing sound
• Korotkoff V is silence
The sounds heard over an artery when blood pressure is determined by the auscultatory method.
Korotkoff are the sounds that medical personnel listen for when they are taking blood pressure using a non-invasive procedure. They are named after Dr. Nikolai Korotkoff, a Russian physician who described them in 1905, when he was working at the Imperial Medical Academy in St. Petersburg.
Description
The sounds heard during measurement of blood pressure are not the same as the heart sounds 'lub' and 'dub' which are due to the closing of the hearts valves. If a stethoscope is placed over the brachial artery in the cubital fossa in a normal person (without arterial disease), no sound should be audible. As the heart beats, these pulses are transmitted smoothly throughout the arteries and no sound is produced.
If the cuff of a sphygmomanometer is placed around a patient's upper arm and inflated to a pressure above the patient's systolic blood pressure, there will similarly be no sound audible. This is because the pressure in the cuff is high enough such that it completely occludes the blood flow. It is similar to a flexible tube or pipe with fluid in it that is being pinched shut.
If the pressure is dropped even with the patient's systolic blood pressure, the first Korotkoff sound will be heard. As the pressure in the cuff is the same as the pressure produced by the heart, some blood will be able to pass through the upper arm when the pressure in the artery rises during systole. This blood flows in spurts as the pressure in the artery rises above the pressure in the cuff and then drops back down, resulting in turbulence that results in audible sound.
As the pressure in the cuff is allowed to fall further, thumping sounds continue to be heard as long as the pressure in the cuff is between the systolic and diastolic pressures, as the arterial pressure keeps on rising above and dropping back below the pressure in the cuff.
Eventually, as the pressure in the cuff drops further, the sounds change in quality, then become muted, then disappear altogether. As the pressure in the cuff drops below the diastolic blood pressure, the cuff no longer provides any restriction to blood flow allowing the blood flow to become smooth again with no turbulence and thus produce no further audible sound.
Korotkoff actually described 5 types of Korotkoff sounds:
1.The first Korotkoff sound is the snapping sound first heard at the systolic pressure.
2.The second sounds are the murmurs heard for most of the area between the systolic and diastolic pressures.
3.The third and-
4.the fourth sound, at pressures within 10 mmHg above the diastolic blood pressure were described as "thumping" and "muting".
5.The fifth Korotkoff sound is silence as the cuff pressure drops below the diastolic blood pressure.
Traditionally, the systolic blood pressure is taken to be the pressure at which the first Korotkoff sound is first heard and the diastolic blood pressure is the pressure at which the fourth Korotkoff sound is just barely audible. However, there has recently (2000 onwards) been a move towards the use of the 5th Korotkoff sound (i.e. silence) as the diastolic blood pressure, as this has been felt to be more reproducible.
An auscultatory gap is the interval of pressure where Korotkoff sounds indicating true systolic pressure fade away and reappear at a lower pressure point during the manual measurement of blood pressure. The improper interpretation of this gap may lead to blood pressure monitoring errors: namely, the falsely low recording of systolic blood pressure.
The auscultatory gap can be mistaken for the silence that occurs when the cuff pressure exceeds the systolic pressure. In this setting, the true systolic pressure can be elucidated by palpating the radial artery pulse. It is therefore recommended to palpate and auscultate when manually recording a patient's blood pressure.
There is evidence that auscultatory gaps are related to carotid atherosclerosis and to increased arterial stiffness in hypertensive patients, independent of age.
Although these observations need to be confirmed prospectively, they suggest that auscultatory gaps may have prognostic relevance.

5.The fifth Korotkoff sound is silence as the cuff pressure drops below the diastolic blood pressure.
Traditionally, the systolic blood pressure is taken to be the pressure at which the first Korotkoff sound is first heard and the diastolic blood pressure is the pressure at which the fourth Korotkoff sound is just barely audible. However, there has recently (2000 onwards) been a move towards the use of the 5th Korotkoff sound (i.e. silence) as the diastolic blood pressure, as this has been felt to be more reproducible.
An auscultatory gap is the interval of pressure where Korotkoff sounds indicating true systolic pressure fade away and reappear at a lower pressure point during the manual measurement of blood pressure. The improper interpretation of this gap may lead to blood pressure monitoring errors: namely, the falsely low recording of systolic blood pressure.
The auscultatory gap can be mistaken for the silence that occurs when the cuff pressure exceeds the systolic pressure. In this setting, the true systolic pressure can be elucidated by palpating the radial artery pulse. It is therefore recommended to palpate and auscultate when manually recording a patient's blood pressure.
There is evidence that auscultatory gaps are related to carotid atherosclerosis and to increased arterial stiffness in hypertensive patients, independent of age.
Although these observations need to be confirmed prospectively, they suggest that auscultatory gaps may have prognostic relevance.

With the right attitude, even losing can make you a better person.
THERE will come a time in every individual’s life when things don’t go our way and this is the reality of life.
One of the most common occurrences is defeat, which comes in various situations in our daily and challenging undertakings and often causes anger, disappointment, depression, unhappiness and discomfort.
At the recent 12th general election, failure was more felt by a greater number of politicians than success and this had left many people with a disturbed mood, shame and pain.
I called two politicians whom I had interviewed during their campaign trail after their defeat was announced and they refused to answer my calls.
Another politician who I went to visit cursed and swore because he lost his seat and was forced to vacate his house hurriedly following the fall of the Selangor government.
A real man is one who can face up to his mistakes and learn something from them so that he never repeats them.
Here credit must be given to former Penang chief minister Tan Sri Dr Koh Tsu Koon who accepted his defeat and the downfall of the state government gracefully and passed on the baton respectfully.
He left with dignity and style and nothing commands respect more than a man who stands up to adversity because respect clears the way to power and success.
Many say this is easily said than done because it is difficult to accept or admit defeat.
One shouldn’t be afraid to acknowledge defeat because an individual who has the ability to stand up tall and admit that he’s wrong or that he’s been bettered in some way is admired and regarded as a gentleman.
This is because with the right attitude, even losing can make you a better person as the entire episode offers us an experience to learn from.
We spend our whole life building on a good character and this is not based on how we win every task we undertake but how we deal with losses and emerge stronger.
People with credible disposition do not fall following a failure but surface greater in the society.
Don’t dwell on your performance which wasn’t the best, instead recognise your defeat. Congratulate the winner and move on with positive thoughts to recover from your setback quickly.
Never hold grudges and take revenge because such thoughts are counterproductive and in fact they will affect your physique and aura.
Swallow your pride and ego and learn from your mistakes and turn your errors to your advantage.
Every loss should be played back in your head again and again to determine exactly why you lost.
This will keep you sharp and focused for the next time you face this particular adversary.
In a defeated situation you often ask why it was him and not you who came out on top. Did he have something that you didn’t?
There’s nothing wrong with admiring those who won and take stock of what it took to come out on top.
Another defeated politician I spoke to attributed his loss to the will of God and has turned to spirituality to heal his body, mind and soul.
Many people turn to the Almighty when in crisis, not realising that they should have done it before so that they have the divine guidance and grace in undertaking challenges in life.
The reality of life is there are super powers above us and all we have to do is give our best in all our undertakings and leave the rest to the unseen supremacy.

Another Fall 4 me... damn... y m i so like tat..
yo... it has been a long time 4 me since my last bloggy. haih... my fren used 2 say, i ve done my part n i ve had it. its a nice quote thou. yt i m still din do any of my part. y m i so like tat.
m i reli tat useless. doubt bout tat. my part is so ez 4 any1 tat reli wish 2 do tat. yt i cant do it properly. damn. how could i become like tat. i can say tat i will nt hv any return time d. tis is my last ticket i dun hv anymore 2 buy. its like u do it or jz 4gt it. i cant afford 2 4gt it. so plz dun b so lz n buckle up. tats wat i told myself in my previous blog. yt i din do it.

New Year is comin...!!! Hooray...!!!
haha... new year is comin.. so happy tis year...
happier thn last year... last year i m havin a sad sad year.. cuz of my own personal reason... anyhow tis year is a wonderful n happiest year i m gonna hav..
in front of me will b another tuff year 2 go thru.. so i muz buckle up n rush thru d followin year in a smooth n splendid one.. muz reli do it...
make tis as my new year resolution... haha.. a piece of advice 2 myself... keep d resolution near 2 me.. dun delay it cuz i dun hv much time of delayin it.. muz keep up2date... haha...
anyhow i muz thnx my parents.. both of thm reli provided me support n care. i m nt gal i cant do much... haha. all i can do is do my best n dun let thm down. muz keep thm out of worryin me. hope for d best of me n others as well... esp my... haha.. i noe can d..

New Year New Beginning...
New Year new beginning. tats wat usualy ppl will alwiz said. it is true thou. u muz at least hv a new year resolution. n 1 thing 4 sure is u muz make it. now i m in a condition like do or die. d feelings is damn suffering. duno wat 2 do everyday thou there is something there 4 u 2 do.
